Terms & Conditions
Last updated: 11/05/2026
These Terms & Conditions govern your use of trishformbyphysio.co.uk (“the website”) and any services we provide through it. By using the website or any of our services you agree to be bound by these terms.
1. About us
Patricia Formby, trading as Trish Formby Physio. Address: Trish Formby Physio, Shore Rd, PO188QL. Contact: hello@trishformbyphysio.co.uk. ICO registration: ZC142986.
Patricia (Trish) Formby is a Chartered Physiotherapist registered with the Health and Care Professions Council (HCPC PH47373) and a member of the Chartered Society of Physiotherapy (CSP 059116).
2. Use of the website
You may use this website for lawful personal use only. You agree not to misuse the website, attempt to gain unauthorised access, introduce malicious code, or use any content in a way that infringes our intellectual property rights or the rights of others.
3. Online classes and educational content — important medical disclaimer
All online classes, exercise programmes, articles and educational content on this website (and within any future paid membership) are provided for general information and self-directed movement only. They are not a substitute for personalised physiotherapy assessment, diagnosis or treatment.
Before starting any new exercise programme — particularly if you have a medical condition, are pregnant or recovering from injury or surgery — you should consult your doctor or a qualified healthcare professional. If you experience pain, dizziness, or any unusual symptom during a class, stop immediately and seek advice.
Participation in any class or programme is at your own risk. You are responsible for ensuring your environment is safe (clear floor space, suitable footwear, appropriate equipment) and that any movement is appropriate for your current level of fitness and any health conditions you may have.
If you have a specific injury, persistent pain, or any concern that needs personal assessment, please book a 1:1 physiotherapy consultation rather than relying on online content alone.
4. 1:1 physiotherapy services
1:1 physiotherapy consultations are provided by Patricia Formby, a Chartered Physiotherapist regulated by the HCPC. They are governed by the standards of practice of the HCPC and CSP. Booking, fees, cancellation and consent for treatment are addressed at the time of booking and during the consultation itself.
5. Online membership
• Subscriptions are provided on a recurring basis (monthly or annually, as selected at signup) and renew automatically until cancelled.
• Fees are stated at signup and may be changed with at least 30 days’ notice. Existing subscribers will be notified by email before any price change takes effect.
• You can cancel your subscription at any time via your account or by contacting us at hello@trishformbyphysio.co.uk. Cancellation takes effect at the end of your current billing period; no refund is given for the unused portion of the period unless required by law.
• We may suspend or terminate access for breach of these terms, fraudulent activity, or non-payment.
• Statutory cancellation rights under the UK Consumer Contracts (Information, Cancellation and Additional Charges) Regulations 2013 apply where applicable. By starting to access digital content during the 14-day cooling-off period, you may waive your right to a refund — this will be made clear at signup.
6. Payments
Payments are processed by our payment provider (Stripe). We do not store full card details. By providing payment information you authorise us to charge the agreed amount on the agreed schedule.
7. Intellectual property
All content on this website — including text, video, audio, images, programmes and class formats — is owned by Patricia Formby or licensed to us. You may view it for personal use only. You may not download, copy, redistribute, share login details, screen-record, broadcast or use any content commercially without our prior written permission.
8. User conduct
You agree not to share your account login or membership content with anyone else; not to record or redistribute classes; not to use the website to harass, abuse, or harm any person; and not to upload any unlawful, defamatory or infringing content.
9. Limitation of liability
Nothing in these terms limits or excludes our liability for death or personal injury caused by negligence, for fraud, or for any other liability that cannot be limited by law.
Subject to that, to the maximum extent permitted by law:
• Our total liability to you in connection with the website, online content and online membership is limited to the total amount you have paid us in the 12 months preceding the claim.
• We are not liable for any loss or damage that is not foreseeable, or for any indirect, special or consequential loss.
• We are not liable for any injury or harm arising from your participation in online classes where you have not followed the safety guidance set out in these terms or in the relevant class.
10. Indemnity
You agree to indemnify us against any losses we suffer as a result of your breach of these terms or your misuse of the website.
11. Changes to these terms
We may update these terms from time to time. The “last updated” date at the top will reflect any changes. Continued use of the website after changes are posted constitutes acceptance of the updated terms.
12. Governing law
These terms and any dispute arising from them are governed by the laws of England and Wales, and you and we agree to submit to the exclusive jurisdiction of the courts of England and Wales.
